For this section, you must go to this website https://www.atsusers.com and complete the following items. It looks like a lot, but it is absolutely necessary and should not take too long to complete. You will not be allowed to participate until all the information is completed.
All Student Athletes: See instructions below for setting up your ATS Electronic Medical Records Account and then complete the following sections.
Log on to https://www.atsusers.com
Click on "ATS Athlete Portal"
Enter the information as follows for a new user:
ID: new
Password: new
Database: atsusi
Once you enter the system you will need to:
Put in your name, background info, team, and change your password (Be sure to write it down!) For your username, please make it your first initial, followed by your last name. (Example: John Smith would be jsmith). You will receive an email with your log-in information and password. Please save this email for future reference. Please use your school email address. Under the “alternate ID” blank, please put your USI-assigned student ID number. (example: 000123456)”
On the main menu, click on athlete information then under general information, complete everything. You MUST put down a permanent address and cell phone number. We do not need your social security #, vtext, twitter tag, blood type, driver's license #, and passport #. This information is not necessary. Make sure you hit SAVE after completing each section!
• Upload a picture (headshot) if you have one. If you do not have a picture, that is fine.
• On the medical history page you need to complete everything. Please note any surgeries.
• On the Insurance page you need to complete everything – to do this you need to click on the edit before it will let you put information in the boxes. In the "Payor" box, put the number 1.
• On the Emergency Contact you need to complete everything – again, click on the edit to type in the boxes. Be sure to include more than one emergency contact in case we cannot get a hold of the first person in the event an accident does occur. The contact order is 1. If you list more than one contact, please put them in the order you would like them contacted.
• On the Athlete Form page, you must complete the Pre-Participation Form AND the Sickle Cell Form. These are two separate forms and both must be completed individually from the drop-down menu.
