Tryout Checklist: USI Cheer Team 2020-21 Tryout Checklist
1. Each applicant will be required to submit the
2020-21 tryout form.
2. Each applicant is required to submit a physical examination at his/her own expense (no blood work or urinalysis required) prior to participation in athletics at USI. Physicals from previous institutions will not be accepted. Also, physicals must be completed by an MD, DO, PA or NP only (NO EXCEPTIONS). Please use the
USI Physical Form.
Please bring your physical to the first night of tryouts. For those individuals who were part of the Cheer Team during previous seasons, you do not need to complete a new physical unless otherwise told. Check out the
USI Sports Medicine/Athletic Training website for additional information.
3. Each applicant must provide a copy of their current medical insurance card.
4. Each applicant will be required to submit the
Waiver/Release Form. If the student is under 18, a parent/guardian signature is needed.
5. Each applicant will be required to review the
Rules and Expectations. No one is expected to sign until they have made the team.
6. Attire.
   —Females will need to wear a white polo, navy shorts, cheer/athletic shoes, game day ready hair and makeup;
   —Males: White polo, navy shorts and athletic shoes.
7. Register for tryouts using the link. $20 tryout fee.
*All of these items are required in order to tryout.
